The program allows individuals with developmental disabilities the opportunity to gain skills and build their natural supports over the course of two years.
  • Transitional In-Home Program: Serves individuals who have acquired brain injury or head trauma after the age of 22 years. Participants are assigned to a service coordinator through the Department of Health. The service includes training and practice with activities related to daily living and maintenance of a household.
