Nursing Home Transitioning: Assists individuals with moving out of the nursing home and back into their own home in the community with initial planning, finding a home, locating and obtaining household items, helping find financial assistance, along with the many other stages of the transitions program.

Transitioning: The objective of the program is to move individuals from a nursing or habilitation facility to the community, identify and eliminate barriers that prevent people from being able to move to the community, improve the ability of MO Health Net to provide in-home services and to ensure that there's continuous quality improvement of in-home services provided.
