- Age 3-19 years with developmental disabilities
- Must be able to sit upright without support
- Weight limit of up to 140 pounds

- Registration Form
- Liability Release Agreement
- Emergency medical Treatment form
- Medical History and Physician Statement form
- If the child receives physical and/or occupational therapies then a Physical/Occupational Therapy Evaluation form should also be submitted
