Eligibility
Resident of Boone County
  • Families with children under the age of 18, adults with disabilities, or persons ages 60 and older
  • Household income must be at or below 150% of the federal poverty level
  • Utility accounty must be in the eligible applicant's name.
Required documents
Must fill out program application completely, including the following documents:
  • Copy of account holder's photo ID
  • Copies of social security cards for all household members
  • Copy of account holder's utility bill
  • Proof of income for previous month: paycheck stub, social security income, unemployment, child support, TANF, disability income, alimony, pension, etc.
  • Working telephone number

  • Applications are available at Health Department office, Boone Electric - Cooperative, and City of Columbia Water and Light Customer Service.
Electronic applications can be found on the City's website at: www.gocolumbiamo.com/CommunityServices/Programs/Social_Service/socserv.php Completed applications and documents may be mailed or dropped off at the Health Department, faxed to (573)874-7756, or emailed to [email protected]. Completed applications will be processed within 30 days. Accepted applications will be eligible for random selection for 12 months from the date the application is accepted.
