Provide intensive case management to those with chronic diseases or who have been in Emergency Room frequently. Diseases can include congestive heart failure, chronic obstructive pulmonary disease (COPD), chronic pain or gastrointestinal problems, high blood pressure, seizures (epilepsy), asthma, diabetes, obesity, renal (kidney) disease or chronic tobacco use. Accept cancer patients IF have other chronic diseases.
