1. Must be in an eligible category: pregnant, breastfeeding, infant, or child (ages 5 and below) 2. At or below 185% of federal poverty based on the income of the entire household. May be automatically eligible if receiving SNAP, TANF, or Missouri Medicaid 3. Be a resident of the state of Missouri by having a Missouri address 4) Services are provided to all residents on a non-discriminatory basis.
Required documents
For certification appointments, must bring the person being certified, proof of income for the household, proof of address, and proof of identity.
