Individuals aged 60 years of age and older or those asking on behalf of someone aged 60 years of age and older
Required documents
Must complete an agency registration form (NAPIS) and assessment forms. Social Security Card, Proof of Residence, Medicare Card might be required to apply for some benefits

Provide Care Coordination to older persons and their family caregivers, including assessment, care plan development, monitoring and assistance in accessing public and private services. Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients experiencing any problems with those programs.
