No bills older than 6 months. Bill must be under $1000. Applicant must pay half the amount being requested. Must not have received assistance on that bill in the last month. Must have tried other agencies first.
Required documents
Photo ID, proof of address, proof of disability, copy of lease, Social Security cards for all household members and proof of income.
