- Individuals asking on behalf of someone aged 60 years and older

Must complete an agency registration form (NAPIS) and assessment forms.
Social Security Card, Proof of Residence, Medicare Card might be required to apply for some benefits

Provide Care Coordination to older persons and their family caregivers, including assessment, care plan development, monitoring and assistance in accessing public and private services. Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients through case/care management to those who are experiencing problems with those programs.
