Application process
MUST BE REFERRED BY AN AGENCY OR PROFESSIONAL, I.E. CLERGY, SOCIAL WORKER, PHYSICIAN, TEACHER, THERAPIST WORKING WITH THE PERSON. Applicant may NOT submit application him/herself.
Professionals can call or email for an application. Submitted applications are screened, and when all information is received, they are presented to the Disbursement Committee which meets weekly to make decisions.
