DOCTOR'S ORDERS/PROVIDER REFERRAL ONLY FOR DIABETIC SUPPLIES & MEDICAL EQUIPMENT. MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY.
Emergency assistance provided for durable medical equipment, medication prescriptions, enzyme prescriptions and eyeglasses. Insulin, eye exams, hearing aids and emergency dental care also provided. Do not pay Medicaid co-pays.
