- Military service of at least 1 day beyond initial training
- Homeless verification which will be conducted by St. Patrick Center

- DD214 (an SF180 can be filled out at assessment if veteran does not have proof)
- Proof of Income
- Driver's license or state identification card
- Social Security Card for all members of the household
- Birth certificate for children in household
