No restrictions, minors must have a parent/guardian present to apply

Proof of Identification (Photo ID or ID Card, passport, military ID, photo school or work ID card, Visa, Resident card), Proof of Address (rent lease or mortgage doc, credit card doc, payroll stub, bank statement, proof of school enrollment for minors, etc.), Application provided by the Health Department If primary identity documents aren't available, they can take 2 forms of alternate identification for Proof of Identification (letter from govt or social service org, school yearbook, W2, court certified adoption docs, auto insurance, etc.)

M-F 8am-5pm (Closes at 1 p.m. every 4th Thursday for staff development.)
