Income guidelines, must have steady income; Assistance once every 12 months; No assistance from another agency in the last 12 months; Maximum assistance $200; If amount owed is over $200 must have paperwork documenting arrangements with landlord or other agencies for the rest of the balance.

Photo ID, copy of disconnection or past due notice, SS cards for all household members, proof of income (if any)
