MUST HAVE MADE A PAYMENT IN THE LAST 90 DAYS, NO OLD BILLS (from a previous address). NO BILLS OVER $1,000. Require income source for electrical assistance, can only receive assistance once every 24 months.

Documentation needed time of appointment, not application: photo ID, proof of income and address, SS cards for family, copy of lease and eviction or late notice
