Eligibility
Currently homeless by HUD definition: Sleeping in a place not meant for human habitation, in a shelter or transitional housing, fleeing domestic violence.
For those who are not currently homeless, a triage screen will be completed and families will be diverted to emergency assistance services and provided case management and related services.

No ID or documents are needed for the screening or completion of the Vulnerability Assessment Tool for entry onto the Prioritized Housing Eligibility List

Call and ask for social services. Outside of office hours, go directly to a community shelter if needed and then call or visit location during the next available open time.
